If you would like to register with the Practice please ask at reception for a registration form.
We require you to provide two forms of identification. One must be a photo ID (e.g. passport or driving licence) and the second an up-to-date utility bill or solicitor’s letter with your name and current address. In addition to the registration form you will be asked to complete a short questionnaire providing details of your medical history and lifestyle. Patients are also offered a new patient check with the Practice’s HCA or, if appropriate, with the nurse or doctor.
If you are on repeat medication it would be helpful if you could bring a printout from your previous surgery, or provide the prescription counterfoil, when you register.
Please note that the Practice does not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation or appearance, disability or medical condition.
Medical treatment is available from the date of registration.
Dr Charles Shlosberg is the named accountable GP for all patients registered at the Practice but you may see any of the doctors working at the surgery.
